Janumet XR/CP Tablets combine two active ingredients, Sitagliptin and Metformin, to effectively manage blood glucose levels in adults with type 2 diabetes. Sitagliptin is a DPP-4 inhibitor that enhances the body’s natural ability to lower blood sugar by increasing incretin levels, which help regulate glucose metabolism. Metformin is a biguanide that improves insulin sensitivity, decreases hepatic glucose production, and enhances glucose uptake and utilization in peripheral tissues.
Indications:
- Management of blood sugar levels in adults with type 2 diabetes mellitus, particularly in combination with diet and exercise.
- Can be used as a second-line treatment in patients not adequately controlled on Metformin alone.
Dosage and Administration:
- The usual starting dose is one tablet (Sitagliptin 100mg + Metformin 1000mg) taken once daily with food to reduce the risk of gastrointestinal side effects.
- Dosage may be adjusted based on blood glucose levels, tolerability, and physician recommendations.
- Patients should adhere to their prescribed diet and exercise regimen to maximize the therapeutic effects.
Contraindications:
- Hypersensitivity to Sitagliptin, Metformin, or any of the excipients.
- Severe renal impairment or acute or chronic metabolic acidosis, including diabetic ketoacidosis.
- Patients undergoing procedures that require contrast media administration should discontinue the use of Metformin before and for 48 hours after the procedure.
Precautions:
- Regular monitoring of renal function is recommended, as Metformin is contraindicated in patients with renal impairment.
- Caution in patients with a history of pancreatitis, as there is a potential risk of developing pancreatitis with DPP-4 inhibitors.
- Alcohol consumption should be limited, as it can increase the risk of lactic acidosis.
- Pregnant or breastfeeding women should consult their healthcare provider before use.
Side Effects:
Common side effects may include:
- Nausea
- Diarrhea
- Stomach pain
- Headache
Serious but less common side effects include:
- Lactic acidosis, a rare but serious condition that can occur with Metformin.
- Pancreatitis, which may present as severe abdominal pain, nausea, and vomiting.
- Hypoglycemia, especially if used with other antidiabetic medications.
Drug Interactions:
- Certain medications may increase the risk of lactic acidosis or affect glucose levels, including diuretics, corticosteroids, and other antihyperglycemic agents.
- Inform your healthcare provider of all medications, supplements, or herbal products being taken to avoid potential interactions.
Storage:
- Store at room temperature, below 30°C (86°F), away from moisture and heat.
- Keep out of reach of children.
Packaging:
Available in blister packs, with each tablet containing 100mg of Sitagliptin and 1000mg of Metformin Hydrochloride.
